29 CFR 1910.147(c)(4)(i):Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Planer mill; stacker hoist; On or about May 18, 2022 and at times prior to the employer exposed employees to struck by hazards in that employees were allowed to conduct maintenance on the stacker hoist without first utilizing lock out tag out.

29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed: a) Planer mill; stacker hoist; On or about May 18, 2022 and at times prior to the employer exposed employees to struck by hazards in that employees were allowed to conduct maintenance on the stacker hoist without first being safely positioned when equipment was re-energized for testing.
