29 CFR 1910.252(a)(2)(xiii)(A): The employer did not recognize its responsibility for the safe usage of cutting or welding equipment, based on fire potentials of plant facilities, and establish areas for cutting and welding, and procedures for cutting and welding, in other areas: On or about May 14th, 2022, employees engaged in the use of oxygen and acetylene cutting torch operations were exposed to explosion hazards as the employer failed to establish areas for cutting and welding procedures.

29 CFR 1910.252(a)(3)(i): Welding or cutting was performed on used drums, barrels, tanks or other containers before they were cleaned so thoroughly as to make absolutely certain there were no flammable materials present or any substances which when subjected to heat or any substances such as greases, tars, acids or other material which when subjected to heat might produce flammable or toxic vapors. On or about May 14th, 2022, employees engaged in using sources of ignition to remove bolts from a nipple valve on tanker trailer were exposed to explosion hazards.
