29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) Building Exterior: An employee was exposed to eye/face injuries while cutting metal with a 4 Inch DEWALT angle grinder without the use of eye and face protection. Employee was not protected with any type of eye/face protection, on or about 5/20/22.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  (a): Exterior pipe scaffold: Employees working on the first and second level of the scaffold were exposed to fall hazards of approximately 6 to 13 feet onto the ground below below due to scaffold deck not being fully planked, on or about 5/20/22.
