29 CFR  1926.451(g)(3)(iv): Vertical lifelines and outrigger support lines were attached to the same point of anchorage:    a) Jobsite located at 305 N 15th St, Philadelphia, PA: Employees performing work from a two-point adjustable suspension scaffold were exposed to 70-foot fall hazards in that anchorage points for two vertical lifelines were installed on the same structural steel beam being used as an anchorage point for the suspension scaffold tiebacks.
