One case was recorded with no indication in Columns G, H, I or J regarding its classification. c. Two cases were recorded with no indication as to the classification in Columns G through J, the number of days away or restricted duty in Columns K and L, or the type of injury or illness in Columns M1 through M6. d. One case with no indication in Columns M1through M6 identifying the type of injury or illness. e. Two cases with no indication in Columns K or L regarding the number of days away or restricted duty.

29 CFR 1910.134(c)(3):The employer did not designate a program administrator who was qualified by appropriate training or experience to administer or oversee the respiratory protection program and to conduct the required evaluations of program effectiveness: COMPANY-WIDE: On or about May 26, 2022, the employer's written Respiratory Protection Program did not designate a program administrator. Employees were required to wear respirators when exposed to infectious airborne pathogens including, but not limited to COVID-19 and tuberculosis.
