29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. Abatement Certification and Documentation is required Facility-wide: On or about May 26, 2022, and ongoing, the employer did not record employee work-related COVID-19 infections on the OSHA 300 log or its equivalent during the following instances: a) A healthcare worker tested positive for COVID-19 on May 20, 2022. The employee had a minimum of fifteen lost workdays. b) A healthcare worker tested positive for COVID-19 on May 20, 2022. The employee had a minimum of seven lost workdays. c) A healthcare worker tested positive for COVID-19 on May 21, 2022. The employee had a minimum of fourteen lost workdays. d) A healthcare worker tested positive for COVID-19 on May 24, 2022. The employee had a minimum of fifteen lost workdays.
