29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a) Exterior of Building - Roof: Employees without any means of fall protection were exposed to fall hazards of approximately 10 feet while engaged in framing activities next to a door opening, on or about 5/14/22.  b) Interior of Building - 2nd Floor - Hallway:  Employees without any means of fall protection were exposed to fall hazards of approximately 10 feet while engaged in framing activities next to a opening without proper railing, on or about 5/14/22.

29 CFR  1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.   a) Exterior of Building - Roof: An employee fell from a job made ladder that did not reach the landing while accessing the roof, on or about 5/14/22.
