Site- On or about May 26, 2022 employees were not protected from electric shock and burns where a Gowone, 32 amp, 220-volt, relocatable power tap was not inspected for defects prior to being placed into service. The relocatable power tap had burn marks on its attachment plug and the 220-volt outlet receptacle that it was plugged into was burned and melted where it had been overheated.   b. Site- Employees were not protected from electric shock and burns where the 220-volt relocatable power taps were not inspected for defects prior to being placed into service. All of the 220-volt relocatable power taps that were in service had the ability to accept a 110-volt attachment plug for 110-volt cord and plug connected equipment.  c. Site- Employees were not protected from electric shock and burns where relocatable power taps operating at 220-volts and 110-volts were not inspected for defects prior to being placed into service. Multiple relocatable power taps were in use with power cords that were damaged and did not provide strain relief at the attachment plug and/or the housing of the relocatable power tap.
