29 CFR 1910.147(a)(2)(ii)(A):Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to remove or bypass a guard or other safety device: On May 12, 2022, at 7361 NW 78 Street, Florida, an employee removing a plastic burr from the mold of the stretch-blow molding machine was exposed to amputation hazards when simultaneously placing his hand at the machine's point of operation while also manipulating bottoms from the microcomputer controller.

29 CFR 1910.147(a)(3)(i): The employer failed to establish a program and utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices and to otherwise disable machines or equipment to prevent unexpected energization, startup or release of stored energy in order to prevent injury to employees. n May 12, 2022, at 7361 NW 78 Street, Florida, an employee removing a plastic burr from the mold of a stretch-blow molding machine was exposed to amputation hazards when the employer had not establish and utilize a procedure to affix lockout or tagout devices.
