29 CFR 1910.212(a)(3)(ii):Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: (a) Right side of pump - On or about May 26, 2022 the employer instructed their employees to perform Semi-annual PM on Pump 35-7 #2 where the "V" belt pully is not properly guarded exposing employees to amputation hazards.
