29 CFR 1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report. a) On or about May 25, 2022, the employer had not used an OSHA 300 Log or equivalent for recording work-related injuries and illnesses for 2021 in that the year, establishment, and location were not present on the log, and multiple establishments were present on the log. The employer had used an e-compensation database which included all years and locations covered by the safety office for Fort Wayne, Indiana. b) On or about May 25, 2022, the employer had not used an OSHA 300 Log or equivalent for recording work-related injuries and illnesses for 2022 in that the year, establishment, and location were not present on the log, and multiple establishments were present on the log. The employer had used an e-compensation database which included all years and locations covered by the safety office for Fort Wayne, Indiana.

29 CFR 1904.32(a)(1): Annual summary. Basic requirement. At the end of each calendar year, you must: Review the OSHA 300 Log to verify that the entries are complete and accurate, and correct any deficiencies identified. a) On or about May 25, 2022, the employer did not review the 2021 OSHA 300 Log to verify the accuracy in that the signed OSHA 300A summary for 2021 and the OSHA 300 Log for 2021 did not contain the same information such as but not limited to cases with days away from work, cases with transfers, recordable cases, lost work days, transfer or restricted days and cases with injuries, respiratory conditions and all other illnesses.
