Location: 15 -16 Village Green Court, South Orange, NJ Employees engaged in roofing activities used a Werner extension ladder to access a rooftop to install new shingles without extending the ladder at least 3 feet above the landing surface. The access point of the roof was approximately 30-feet to the ground. Violation observed 05/23/22
