29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Location - 30 Farm Road, Ipswich, MA: On or about 5/26/2022, the employer allowed employees to be hoisted by a rough terrain forklift while working from an elevated platform of lumber and plywood supported on the forks. The design and use of the elevated platform was not in compliance with ANSI/ITSDF B56.6-2021, 5.17 requirements for elevating personnel and/or approved by the powered industrial truck manufacturer.

The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated. Location - 30 Farm Road, Ipswich, MA: On or about 5/26/2022, employees working from an aerial lift were not protected from ejection hazards where personal fall restraint equipment was not worn or connected to the basket of the lift.
