29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a)	This violation occurred on or about May 31, 2022, located at the 500 Singleton Blvd, corner of Bataan and Singleton Blvd, Dallas, TX; where employees installing fiberoptic cables in the ground. The employer does not ensure employees recognize, control or eliminate unsafe hazards while working at the job site.

29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)	This violation occurred on or about May 31, 2022, located at the 500 Singleton Blvd, corner of Bataan and Singleton Blvd, Dallas, TX; where employees installing fiberoptic cables in the ground while using a Vermeer horizontal directional drilling machine, D16X20 series II/D20X22 Series II, serial no. 101 with a 42-inch pipe wrench at the end of the drill and were exposed to head injuries where a struck by hazard exists and not wear hard hats.

29 CFR  1926.102(a)(1):Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:     (a)	This violation occurred on or about May 31, 2022, located at the 500 Singleton Blvd, corner of Bataan and Singleton Blvd, Dallas, TX; where employees installing fiberoptic cables in the ground while using a Vermeer horizontal directional drilling machine, D16X20 series II/D20X22 Series II, serial no. 101 with a 42-inch pipe wrench at the end of the drill.  Employees were exposed to eye injuries by not wearing eye protection.

29 CFR  1926.651(k)(1):Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions.  An inspection were not conducted by the competent person prior to the start of work and as needed throughout the shift.  Inspections were not made after every rainstorm or other hazard increasing occurrence.  These inspections are only required when employee exposure can be reasonably anticipated:  (a)	This violation occurred on or about May 31, 2022, located at the corner of Bataan and Singleton Blvd, 500 Singleton, Blvd, Dallas, TX; where employees dug a trench that had not been inspected by a competent person for hazardous conditions.

After that time the design need not be at the jobsite, but a copy were not make available to the Secretary upon request:  (a)	This violation occurred on or about May 31, 2022, located at the corner of Bataan and Singleton Blvd, 500 Singleton, Blvd, Dallas, TX; where employees dug a trench where the design was not constructed and maintained.
