29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: Employees engaged in residential framing activities at 11460 Mercury St, Papillion, Nebraska were exposed to fall hazards in that the forklift supported scaffold platform being used was not attached to the forklift forks.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: Employees engaged in residential framing activities at 11460 Mercury St, Papillion, Nebraska, were exposed to fall hazards in that they were working from a height of 12 feet above the ground and they were not using fall protection. MJ Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1522987, citation number 1, item number 1 and was affirmed as a final order on May 21, 2021, with respect to a workplace located at 11858 South 112th Street, Papillion, Nebraska.
