29 CFR 1926.451(b)(1): Each platform on all working levels was not fully planked or decked between front uprights and the guardrail supports: JOB SITE: 3525 Nebraska Avenue, Saint Louis, MO 63118, on or about 5/31/2022, workers were observed on a tubular welded scaffold, working from two (2) levels of the scaffold, approximately 10' 9" and 15' 10" respectively. The working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports. The workers were exposed to a fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: JOB SITE: 3525 Nebraska Avenue, Saint Louis, MO on or about 5/31/2022, workers were permitted to climb on and off the tubular weld scaffolding to access and egress the scaffold platforms without a portable, hook-on, attachable ladders or ramp. The workers were exposed to a fall hazard.
