29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person: a) C.G.R.S., Inc, 8650 Zuni, Thornton, CO 80260: On or about 5/25/2022, the employer did not ensure that employees were protected from slip hazards while in an excavation. An employee was cutting with a saw while standing on a clay dirt surface, and a water line released water onto the surface making the working surface slippery. The employer failed to detect and prevent changing work conditions to protect employees. This condition exposed employees to slip and fall and struck-by hazards.
