On a residence, located at 413 Brookgreen way, lot 802, in Saint Augustine FL, 32092: On or about May 24, 2022, the employer failed to train employees on the recognition of fall hazards while using personal fall arrest equipment, in that framing employees did not know what an approved anchor device is and did not know how to properly adjust lanyard length.
