The employee was exposed to a fall of over 23 feet and was not provided with any type of fall protection.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder: Jobsite located at 1812 Carriage House, 731 Hopmeadow Street, Simsbury, Connecticut: On, or about, May 31, 2022, an extension ladder that was being used to access the roof of the building was positioned across a driveway. The base of the ladder was exposed to vehicular traffic and precautions had not been taken to prevent the ladder from being hit and/or dislodged.
