29 CFR 1904.32(a): The employer did not ensure that at the end of the year that entries were complete and accurate; and an annual summary of injuries and illnesses on the OSHA 300 Log was created, certified and posted. a) USMS Fairbanks - On or about June 1, 2022, a senior establishment management official, head of the agency for which the senior establishment official works, or a management official who is in the direct chain of command between the senior management official and the head of the agency did not ensure illness and injury entries were complete and accurate, and an annual summary of injuries and illnesses on the OSHA 300 logs was created, certified and posted for calendar years 2021, 2020, and 2019, resulting in materially impairing the understandability of the injuries and illnesses in the workplace.
