29 CFR 1910.176(a):Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked. a) On 5/20/2022 the employer did not ensure that aisles and passageways were permanently marked to separate forklift traffic and employees in the process of stocking the racking systems.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) On 5/20/2022, an employee was hospitalized for a work related injury and the employer did not report a reportable injury to OSHA within the required twenty-four hours, in that the hospitalization was not reported to OSHA until 5/26/2022.
