29 CFR  1910.134(c)(1):  A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  (a) SNH CO Tenant LLC, DBA La Villa Grande Care Center,2501 Little Bookcliff Dr., Grand Junction, CO 81501:  On or about March 29, 2022 the establishment did not have a written respiratory protection program while requiring employees to wear an N95 respirator if caring for a COVID-19 positive patient.

29 CFR  1910.134(m)(4):  Written materials required to be retained under this paragraph shall be made available upon request to affected employees and to the Assistant Secretary or designee for examination and copying:  (a) SNH CO Tenant LLC, DBA La Villa Grande Care Center,2501 Little Bookcliff Dr., Grand Junction, CO 81501:  On or about March 31, 2022 the employer did not provide records of the fit tests administered to employees  upon request of the Assistant Secretary.
