29 CFR 1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter (Construction Reference 1926.103(a)): On or about May 25, 2022, at a worksite located at 2700 S Ashland Avenue, Green Bay, WI, an employee wearing a half-mask cartridge respirator had not been fit tested since January 26, 2018. The employee was required to wear respiratory protection while wet cutting a concrete floor inside a building.

29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means: On or about May 31, 2022, at a worksite located at 2700 S Ashland Avenue, Green Bay, WI, an energized electrical outlet on the east wall of the Old Parts Office was missing the cover plate.
