On or about June 2, 2022, at jobsite located in 921 S. Madison St., Hinsdale, IL 60421, employees were exposed to fall hazards in excess of 15 feet while engaged in residential siding activities. Fall protection was not being utilized while performing siding activities. No abatement certification or documentation is required for this item.
