29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated  a. 300 Washington Pl, Hasbrouck Heights, NJ/ Building Front: An employee working from a JLG Aerial Lift was exposed to a fall hazard of approximately 15 feet without the lanyard attached to the aerial lift lanyard anchorage point, on or about 6/01/2022.

29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) 300 Washington Pl, Hasbrouck Heights, NJ/ Building Front: An employees was exposed to a 12 foot fall hazard while  putting a tarp a over a 14/12 pitch roof  without the use of a personal fall arrest system or other means of fall protection, on or about 6/1/2022.  b) 300 Washington Pl, Hasbrouck Heights, NJ/ Building Rear: Three employees were exposed to a 15 foot fall hazard  walking on a 8/12 pitch roof to repair a leak  without the use of a personal fall arrest system or other means of fall protection, on or about 6/1/2022.
