29 CFR  1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:   (a) J&B Construction at 1215 N Main St. Lamar, CO 81052: On or before 6/1/22 the creating, correcting, and exposing employer did not protect employees from injury in that a Milwaukee battery-powered circular saw was missing its adjustable guard. This condition exposed employees to laceration and amputation hazards.   Abatement Note: Abatement Certification is required for this item. 29 CFR  1926.451(a)(6): Scaffolds were not designed by a qualified person:   (a) J&B Construction at 1215 N Main St. Lamar, CO 81052: On or before 6/1/22 the creating, correcting, and exposing employer did not protect employees from injury in that a pallet used by employees to access the roof of a 3-story hotel and transport their materials was not designed or constructed by a qualified person. The pallet was approximately 12x12 feet while the forklift tines were 4 feet long. The pallet was not secured to the forklift. These conditions exposed employees to fall hazards up to 30 feet. 29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a) J&B Construction at 1215 N Main St. Lamar, CO 81052: On or before 6/1/22 the creating, correcting, and exposing employer did not protect employees from injury in that employees rode a wooden pallet up and down to access the roof of a 3-story hotel. The pallet lacked guard rails and or other approved methods of fall protection. These conditions exposed employees to fall hazards up to 30 feet. 29 CFR  1926.1053(b)(13): The top or top step of a stepladder was used as a step:  (a) J&B Construction at 1215 N Main St. Lamar, CO 81052: On or before 6/1/22 the creating, correcting, and exposing employer did not protect employees from injury in that framing employees accessed a catwalk in the attic using a 10 foot step ladder, which was below the level of the catwalk. Employees used the top step and the top of the ladder to access the catwalk. This condition exposed employees to a fall hazard of 10 feet.
