29 CFR 1910.178(n)(4):Industrial truck driver(s) were not required to travel with the load trailing whenever the load obstructed forward view: (a) Eastside of VT3 Building (Outside Smoking Area): On or about 4/13/2022, and at times prior the employer exposed employees to struck-by hazards in that the power industrial truck operators were not required to travel in reverse when transporting A -frame pallets with countertops that obstructed their view to the Plant Distribution Center (PDC).

29 CFR 1910.178(n)(6):Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel: (a) Eastside of VT3 Building (Outside Smoking Area: On or about 4/7/2022, and at times prior, the employer exposed employees to a struck-by hazard, in that employees were not required to keep a clear view of travel when transporting countertops on an A-frame pallet to the Plant Distribution Center (PDC).
