29 CFR 1910.134(m)(2)(i)(C): The employer did not establish a record of the qualitative and quantitative fit tests administered to an employee which included the specific make, model, style and size of respirator tested: Avenues at Arcadia Springfield, LLC located at 525 S Martin Luther King Drive in Springfield, IL. On or about June 2, 2022, the employer had previously conducted fit testing for employees required to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus and the employer did not maintain a record of the specific model, style and size of respirator tested.
