29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  (a) Front of building, during EIFS work:   On or prior to 6/3/22, employees were working from the first level of a tubular welded frame scaffold system approximately 6 feet above a lower level; work platforms were not fully planked.  b) Front of building, during EIFS work:   On or prior to 6/3/22, employees were working from the  second level of a tubular welded frame scaffold system approximately 13 feet above a lower level; work platforms were not fully planked.  Putnam County Wall Systems was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 1415120, citation number 1, item number 1 and was affirmed as a final order on 10/23/19, with respect to a workplace located at 255 Hoppers Lane, Paramus, NJ 07652.

29 CFR  1926.451(g)(1):Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level.  (a) Front of building, during EIFS work:   On or prior to 6/3/22, employees were not provided with fall protection while working from the second level of a tubular welded frame scaffold system (12-13 feet above a lower level).  Putnam County Wall Systems was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1415120, citation number 1, item number 2 and was affirmed as a final order on 10/23/19, with respect to a workplace located at 255 Hoppers Lane, Paramus, NJ 07652.

29 CFR  1926.404(b)(1)(i):Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.  (a) Front of building, during EIFS work:  On or about 6/3/22, extension cord(s) used to power a mixer drill were not protected by ground fault circuit interrupter(s), exposing employees to a shock hazard.
