29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) On the roof of building: On or about 6/3/2022, the employer did not provide training for employees to recognize fall hazards while working on a low-pitched roof, approximately 18-feet above the lower level, as well as the methods necessary to implement adequate protection.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) On the roof of the building: On or about 6/3/2022, employees were exposed to fall hazards while performing work on a low-pitched roof, approximately 18-feet above the lower level, without any means of fall protection. A & M Sheet Metal & Roofing Inc. was previously cited for a violation of this occupational health and safety standard or its equivalent standard (1926.501(b)(11)), which was contained in OSHA inspection number 1276635, citation number 1, item number 1 and was affirmed as a final order on 6/11/2018, with respect to the workplace located at 1517 Old Louisquisset Pike, Lincoln, RI 02865.
