29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  At the jobsite, employees operating  Hitachi nail guns while installing roofing shingles to the roof were not wearing eye protection, exposing the employees to the hazards of being struck by flying objects. 29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system.  At the jobsite, employees installing roofing material on the roof were wearing fall protection but were not tied off exposing the employees to an unprotected fall hazard at a height of over 6 feet to the ground below. 29 CFR  1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section:     Note:  Circumstances where retraining is required include, but are not limited to, situations where (1) Changes in the workplace render previous training obsolete; or (2) Changes in the types of fall protection systems or equipment to be used render previous training obsolete; or (3) When the employer has reason to believe that any affected employee does not  have the understanding and skill required;    At the jobsite, where employees performing roofing activities were exposed to fall hazards up to approximately 20 feet and did not demonstrate hazard recognition and avoidance to fall hazards at heights over 6 feet to the ground below.
