29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a) Front of Building: Employees were exposed to fall hazards of approximately 13 feet while working on a scaffold that was not fully planked, on or about 6/3/22.

29 CFR  1926.451(c)(1)(ii): Guys, ties, and braces were not installed according to the manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations every 26 feet or less for scaffolds greater than 3 feet wide:   a) 4 Day Hill Trail, Totowa NJ 07512: Employees were working on a pipe scaffold that was not secure as per manufactures recommendations or at the closest horizontal member to the 4:1 Ratio, on or about 6/3/22.  Tubular welded scaffold being use was 3 feet wide, the scaffold height was 19 feet height>

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    a) Building Exterior: Employees were using pipe scaffold frames and cross braces to access the scaffold, exposing employees to a fall of approximately 13 feet to the ground surface, on or about 6/3/22
