29 CFR  1910.110(b)(6)(i): Containers and first stage regulating equipment of liquid petroleum gases were not located outside of the building:  (a) Summit Food Service at 500 East 52nd Street North, Sioux Falls, SD 57104: On or about and at times prior to June 6, 2022, the employer did not ensure that containers of Liquefied Petroleum Gases (LPG) were stored outside of buildings. This condition exposed employees to fire and explosion hazards.

29 CFR  1910.303(g)(1)(i)(B):For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater:  (a) Summit Food Service at 500 East 52nd Street North, Sioux Falls, SD 57104: On or about and at times prior to June 6, 2022, the employer did not ensure unimpeded access to five adjacent electrical panels on the north wall of the main warehouse building.  This condition exposed employees potential shock and electrocution hazards related to inability to disable power during an emergency.  (b) Summit Food Service at 500 East 52nd Street North, Sioux Falls, SD 57104: On or about and at times prior to June 6, 2022, the employer did not ensure unimpeded access to an electrical panel on the southwestern wall of the main warehouse building.  This condition exposed employees potential shock and electrocution hazards related to inability to disable power during an emergency.

29 CFR  1910.178(q)(7): Industrial trucks were not examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination was not made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found were not immediately reported and corrected.  (a) Summit Food Service at 500 East 52nd Street North, Sioux Falls, SD 57104: On or about and at times prior to June 6, 2022, the employer did not ensure that forklifts were inspected daily prior to being placed in service.   This condition exposed employees to struck-by hazards or thrown from hazards from operating in and around uninspected forklifts.
