29 CFR  1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means:   a) Job site, 461 Clairton Boulevard, Pleasant Hills, PA: On or about June 1, 2022, during stucco/IEFS related work activities, the employer did not ensure that the scaffolding, which was determined to be approximately 19 feet high and under under 4 feet in width, was properly protected from tipping over such as by restraints, guys or bracing.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used and cross bracing was permitted to be used as a means of access:    a) Job site, 461 Clairton Boulevard, Pleasant Hills, PA: On or about June 1, 2022, during stucco/IEFS related work activities, the employer did not ensure that a safe means of getting onto and off of a scaffold, at a height of at least approximately 12 feet, was provided in that no ladders, stairs or other similar equipment was provided and an employee was allowed to freely climb up/down the scaffold on it's various components to include cross bracing.

29 CFR  1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:     a) Job site, 461 Clairton Boulevard, Pleasant Hills, PA: On or about June 1, 2022, during stucco/IEFS related work activities, the employer did not ensure that scaffolding had a guardrail systems along the open ends, exposing an employee to a fall hazard of up to approximately 12 feet.

29 CFR  1926.451(g)(4)(xv): Crossbracing was permitted to be used as both a toprail and midrail where the crossing point of two braces was between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform:   a) Job site, 461 Clairton Boulevard, Pleasant Hills, PA: On or about June 1, 2022, during stucco/IEFS related work activities, the employer did not ensure that proper fall protection was provided along the length of a scaffold in that a single cross brace was used for fall protection and it's center point was determined to be approximately 44 inches above the platform, with no midrail was in use, exposing an employee to a fall hazard of up to approximately 12 feet.
