29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about 06/06/2022, at 3140 Tamarind Drive in Lauderdale Lakes, Florida, employees engaged in roofing activities were exposed to an approximated 18- to 20-foot fall hazard without using a means of fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: On or about 06/06/2022, at 3140 Tamarind Drive, Lauderdale Lakes, Florida, employees engaged in roofing activities were exposed to a 18-20 foot fall hazard when using a ladder which did not extend 3 feet beyond the landing. the ladder was used on the second level to access the roof , it extended less than 1 foot to upper landing.
