29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter: a) Stratford Manor Rehabilitation and Care Center, 787 Northfield Ave., West Orange, NJ 07052 The employer provided and required employees, including but not limited to Certified Nurse's Assistants (CNAs), to wear N-95 respirators. CNAs were required to provide direct care including, but not limited to feeding, bathing and washing, to confirmed positive COVID-19 residents. The employees were not fit tested annually. Violation occurred on or about June 6, 2022.
