On or about June 14, 2022 at Vitex Extrusions, Franklin, NH.  Multiple eyewash stations were not available for immediate emergency use. Instance 1.  The eyewash station had metal beams approximately 6.5 inches to 8.5 inches in height on secured to the floor, along two adjoining sides creating a tripping hazard, preventing immediate use of the eyewash station. Instance 2.  The eye wash station was block from immediate use by a pallet of materials being stored underneath it.
