29 CFR  1910.28(b)(6)(i):  The employer did not ensure that each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard:  On or about May 26, 2022, through June 2, 2022, employees were exposed to unprotected fall hazards into Melter #1, Melter #2, and Melter #3, containing molten iron at temperatures in excess of 2,000 degrees Fahrenheit, across three shifts daily at the employer's Mapleton, Illinois, facility.  These exposures included the following employee work activities:  a.	Taking temperature readings, b.	Taking button samples,  c.	Taking thermal cups, and  d.	Adding alloy bags directly to the melters.
