On or about June 7, 2022 and times prior there to, the employer did not ensure electrical equipment, relocatable power taps and electrical cords were being used in accordance with instructions in the listing and labeling in Warehouse 2. Electrical equipment was connected to a power source via a "daisy chain" configuration.
