McClean Affiliates, located at 400 North Main Street, Bristol, CT 06010: The employer did not record employee work-related COVID-19 infections on the OSHA 300 log or its equivalent on the following occasions: a) A healthcare worker tested positive for COVID-19 on June 2, 2022, when multiple residents were confirmed COVID-19 positive. Worker had a minimum of five lost workdays. b) A healthcare worker tested positive for COVID-19 on June 5, 2022, when multiple residents were confirmed COVID-19 positive. Worker had a minimum of four lost workdays. Abatement Certification and Documentation is required
