29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about June 7, 2022, in building 2, Mazak QT 300 CNC Lathe interlocks were altered, exposing employees to rotating parts. b) On or about June 7, 2022, in building 4, Haas VF CNC Mills interlocks were bypassed, exposing employees to rotating parts. c) On or about June 7, 2022, in building 5, Mazak QT Smart 250 Lathe interlocks were altered, exposing employees to rotating parts.
