29 CFR  1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) Facility Wide: On or about June 7, 2022, employees were exposed to struck-by and caught-in hazards on Line  8, a horizontal thermoplastic injection molding/extruding machine, because the  employer has failed to establish a program for machine specific energy control procedures.  Abatement certification and documentation required.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) covered the following elements, however, the actions were not done in sequence as required by 29 CFR 1910.147 (d)(1)-(6):  a) Line 8 Hopper: On or about May 31, 2022, an employee troubleshooting the natural feed hopper on Line 8, was exposed to amputation hazards while performing maintenance without applying energy isolation procedures.  Abatement Certification and documentation required.

Abatement certification not required.
