29 CFR 1910.304(b)(3)(ii)(C)(3):Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug were not visually inspected before use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage, and equipment found damaged or defective was not removed from use until repaired: On 6/7/2022 at the Oreillys Distribution center, charging cords for the electric pallet jacks were not inspected before use therefore employees were exposed to electrical hazards from 4 different forklift charging cords that had exposed wires at the cord plugs.
