29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) Mosher Enterprises, Inc. at Centauri High School in La Jara, CO: On or before 6/1/22 the employer did not protect employees from injury in that a cover plate for a junction box was not reinstalled and exposed energized wires were not capped. The junction box contained a 277V lighting circuit. An employee working from a step ladder was shocked as a result and fell off the ladder and was hospitalized. This condition exposed employees to electric shock and fall hazards. Abatement Note: Abatement certification and documentation are required for this item. See enclosed "Certification of Corrective Actions Worksheet."

29 CFR 1904.33(a): At this establishment, the employer did not retain (save) the OSHA 300 Log, the privacy case list (if one exists), the annual summary, and the OSHA 301 Incident Report forms for five (5) years following the end of the calendar year that these records covered: (a) Mosher Enterprises, Inc. at Centauri High School in La Jara, CO: On or before 6/1/22 the employer did not retain OSHA 300A summaries for five years as required. Abatement Note: Abatement certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."
