29 CFR  1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section 29 CFR 1910.147(c)(4)(ii)(A) - (D)  On or about May 27, 2022, the employer did not ensure that its machine-specific procedure for the Pistorius Machine Co. manufactured chop type saw Model No. DCH 16P Serial No. 118561 addressed all hazardous energies such as, pneumatics and gravity during setup when employees' hands and fingers were directly under the running saw blade to make adjustments. The saw heads were not blocked against the unexpected lowering which exposed employees' hands and fingers to hazards likely to result in serious injury.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about May 27, 2022, employees performing setup on the Pistorius Machine Co. manufactured chop type saw Model No. DCH 16P Serial No. 118561 had their hands and fingers exposed to contact with rotating saw blades when the saw head lowered due to the unexpected release of pneumatic energy and gravity.  The employer failed to implement energy control application steps as the machine was not isolated from all energy sources [per the 1910.147(d)(3) requirements]. As a result, the remaining applicable energy control elements, involving LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
