Throughout the facility: On June 8, 2022, and times prior, the employer exposed employees to respiratory hazards, in that a site specific written respiratory protection program was not developed for employees who are required by the employer to wear an N95 respirator while working within the facility. The Providence SNF Operator, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(c)(1) which was contained in OSHA inspection number 1488657, citation number 1, item number 1a and was affirmed as a final order on October 27, 2020, with respect to a workplace located at 1011 South Green Street, Thomaston, GA 30286.
