At Mondi Bags, USA, LLC., located at 281 Hemphill Blvd, Eastman Ga. 31023, the employer exposed employees to caught in and amputation hazards in that the employer did not document hazardous energy control procedures for use by employees assigned to perform servicing or maintenance tasks on the production equipment to include the Tuber machines, and all other production machines.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. At Mondi Bags, USA, LLC., located at 281 Hemphill Blvd, Eastman Ga. 31023, the employer exposed employees to caught in and struck by hazards in that the AM 8115 Line 3 Pusher Arm Accumulator was not properly guarded.
