29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system.  At the jobsite, employees installing roofing material on the roof were not wearing fall protection, exposing the employees to an unprotected fall hazard at a height of over 6 feet to the ground below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:      At the jobsite, employees installing roofing material on the roof accessed and egressed the upper landing surfaces by extension ladders that were not extended at least 3 feet exposing the employees to an fall hazards from ladders up to 20 feet to the ground below.

29 CFR  1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section:     Note:  Circumstances where retraining is required include, but are not limited to, situations where (1) Changes in the workplace render previous training obsolete; or (2) Changes in the types of fall protection systems or equipment to be used render previous training obsolete; or (3) When the employer has reason to believe that any affected employee does not  have the understanding and skill required;    At the jobsite, where employees performing roofing activities were exposed to fall hazards up to approximately 26 feet and did not demonstrate hazard recognition and avoidance to fall hazards at heights over 6 feet to the ground below.

29 CFR  1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.(a) (LOCATION) (IDENTIFY SPECIFIC OPERATIONS(S) AND/OR CONDITIONS)Note:  29 CFR 1926.21(b)(2) may be cited as an additional violation to support this requirement.    At the jobsite, where employees performing roofing activities were using ladders to access and egress the upper landing surfaces did not show hazard recognition and how to avoid the hazards.  A training program and formal training was not provided.
