Except as provided elsewhere in this section or by other subparts of this part, the employer did not ensure each employee on a walking working surface 4 feet (1.2 m) or more above a lower level is protected from falling by one of the systems listed in 1910.28(b)(15)(i)-(iii). a. On or about 06/09/2022, at the job site - employees required to work on the conveyor belt maintenance platforms are exposed to a 9 feet 4 inch fall hazards, in that, employees are required to leave the confinement of the guard rails while climbing over the conveyor belt to reach the other side maintenance platform because there is no safe access.

On or about 06/09/2022, In the electrical room, employees were exposed to electrical contact hazards, in that, the electrical panels were blocked by stored items which prevented the 3' minimum clearance from being maintained.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a. On or about 06/09/2022, at the panel behind ball mill #3, employees were exposed to electrical contact hazards, in that, there were live electrical conductors sticking out of pull boxes and not guarded from accidental contact.
