29 CFR 1910.1200(e)(1):The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: Employees are exposed to chemical hazards. Employees engaged in stucco work were exposed to injury/illness from dermal, ingestion, and inhalation hazards from exposure to hazardous chemicals. The employer had not developed or implemented a written hazard communication program to ensure that employees were aware of the hazards associated with the chemicals with which they work. Hazardous chemicals used at the site included but were not limited to Quikrete Stucco mix.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: Employees were exposed to chemical hazards. Employees engaged in stucco work were exposed to injury/illness from dermal, ingestion, and inhalation hazards from exposure to hazardous chemicals. Employees performing stucco work had not been provided training to recognize, evaluate, and control exposure to hazardous chemicals. Hazardous chemicals used on site included but were not Quikrete Stucco mix.

29 CFR 1926.501(c): When employees were exposed to falling objects the employer did not require employees to wear hard hats and did not implement measures to prevent objects from falling from higher levels as set forth in (c)(1)-(3) of this section: Employees were exposed to struck-by hazards. Employees were working/walking at the lower level of the structure while other employees were working overhead. Employees on the lower level were not provided with hard hats.
